Semen/Embryo Release Form

  • Submissions must be made 24 hours prior to shipping/transfer date.
  • Once submissions have been made, we will send a confirmation email to the email on file. Once you received the email, please respond confirming all information is correct.
  • Your order is not complete until we receive confirmation email back.
  • Releases will be voided if not claimed within 14 days.
  • Any submissions after regular business hours will be addressed the following business day.
  • When releasing inventory to another person the owner understands that if the release is not claimed within 14 days, the release will be voided.
